Public forms vs. Patient Portal

Public forms on this site are for basic inquiries only (name, phone, email, short message). Do not submit PHI (medical details, diagnosis, test results, etc.) through public forms or standard email.

Use the Patient Portal for messages, refills, results, and visit-related information.

Appointments, telehealth, and cancellations

Online booking and requests are provided for convenience and are not guaranteed until confirmed.

  • We may adjust or reschedule based on clinician availability.
  • Telehealth is available for certain concerns; your clinician will advise when an in-person visit is needed.

To cancel or change, follow the instructions in your confirmation or call (480) 943-1220. Missed-visit policies may apply and will be provided at scheduling.

Insurance and billing (high-level)

We work with many plans. Coverage varies by plan and service. Before your visit, confirm your benefits, copays, coinsurance, and deductibles with your insurer. Any cost estimates are informational and not a promise of coverage or final charges.

Your privacy

Two documents apply to privacy:
